Indication of Flugal 50mg Capsule

Flugal 50mg Capsule is prescribed for the treatment of various fungal infections, including hepatosplenic candidiasis and other Candida infections, cryptococcal infections, and secondary prophylaxis for cryptococcal meningitis in individuals with AIDS. It is also used as antifungal prophylaxis in allogeneic bone marrow transplant recipients. Additionally, Fazol 100Mg Tablet is indicated for oral or vaginal candidiasis that is unresponsive to nystatin or clotrimazole, as well as non-life-threatening Candida infections such as cystitis and esophagitis. It is an alternative treatment for those unable to tolerate amphotericin B.

Pharmacology

Fluconazole is an antifungal medication that works by inhibiting the activity of the cytochrome P450 enzyme, which is essential for the synthesis of ergosterol, a vital component of the fungal cell membrane. By interfering with ergosterol synthesis, Fluconazole disrupts the formation of the cell membrane in susceptible fungi, leading to their growth inhibition and ultimately causing their death. Dosage & Administration of Flugal 50mg Capsule

Adult Dose:

• Oropharyngeal Candidiasis: 200 mg PO on Day 1, THEN 100 mg qDay. Treatment should be continued for at least 2 weeks to decrease the likelihood of relapse.
• Esophageal Candidiasis: 200 mg PO on Day 1, THEN 100 mg qDay; doses up to 400 mg/day may be used based on the patient's response. Treat for a minimum of 3 weeks and for at least 2 weeks following resolution of symptoms.
• Vaginal Candidiasis; Candidal Balanitis: Uncomplicated: 150 mg PO as a single dose. Complicated: 150 mg PO q72hr for 3 doses. Recurrent: 150 mg PO quay for 10-14 days followed by 150 mg once weekly for 6 months.
• Cutaneous Candidiasis; Dermatophytosis; Pityriasis Versicolor: 50 mg once daily for up to 6 weeks.
• Systemic Candidiasis; Cryptococcal Infections: Initially, 400 mg, followed by 200-400 mg once daily. Treatment duration is based on clinical and mycological response but is usually at least 6-8 weeks in cryptococcal meningitis. To prevent relapse after a primary course of treatment for acute cryptococcal meningitis in AIDS patients: 100-200 mg daily.
• Prophylaxis of Fungal Infections in Immunocompromised Patients: 50-400 mg daily. Prophylaxis in patients undergoing bone marrow transplantation: The recommended daily dosage is 400 mg, once daily. Patients who are anticipated to have severe granulocytopenia (less than 500 neutrophils per cu mm) should start Fluconazole Injection, USP prophylaxis several days before the anticipated onset of neutropenia, and continue for 7 days after the neutrophil count rises above 1000 cells per cu mm.

Child Dose:

• Child: PO 3–12 mg/kg/day, max 600 mg/day q24h. Max 800–1,000 mg/day may be used for some CNS fungal infections.

Renal Dose:

• Renal impairment: Hemodialysis patients: Usual dose given after each session. CrCl (ml/min) Dosage Recommendation: <50 and not receiving dialysis 50% of the usual dose. >50 Usual dose.

• Drug-Drug Interaction: Flugal 50mg Capsule may interact with drugs used to treat mental illness (pimozide, quetiapine), anti-depressants (reboxetine), antibiotics (erythromycin), migraine treating drugs (ergotamine), heart-related drugs (amiodarone), blood thinners (warfarin), anti-seizure medicines (carbamazepine), blood pressure-lowering agents (losartan), cholesterol-lowering drugs, and anti-diabetic drugs (metformin).
• Drug-Food Interaction: There are no specific studies indicating food interactions with Flugal 50mg Capsule. However, it is advised to consume a diet rich in non-starchy vegetables and gluten-free foods and limit alcohol and caffeine intake.
• Drug-Disease Interaction: Before taking Flugal 50mg Capsule, inform your doctor if you have a medical history of allergic reactions to medicines, heart diseases, liver or kidney problems, blood disorders like acute porphyria, and low levels of magnesium and potassium.

Contraindications

• If you are allergic to fluconazole or any other ingredient of the Flugal 50mg Capsule.
• If you are taking medicines like Astemizole, Terfenadine, Cisapride, Pimozide, Quinidine, Erythromycin.

Pregnancy & Lactation
